Criteria and Application for Coaching Awards:
Check the criteria to see if you are eligible to receive an award. If you are, fill out the information on the Awards Application “Achievement Awards” and send it to the CHSCA office or SUBMIT IT ONLINE. If you are eligible for a level 4 achievement ring, you must include your ring size.
